Company Overview
Neighborhood Health Care, Inc., dba Neighborhood Family Practice (NFP) was founded in 1980 in response to a concern that residents of Cleveland's near west side lacked access to primary health care. NFP focuses on providing high quality primary care in a medical home setting. Our Mission is to partner with the community for everyone's best health.
As a community provider, we've established medical offices in convenient neighborhood locations: Ridge Road, Tremont, Detroit Shoreway, Bellaire-Puritas and West 117th. Our services spans 12 neighborhoods across Cleveland's west side and is comprised of a very diverse mix of residents. NFP serves the largest Hispanic communities in Ohio and provides care to the rising number of refugees resettling in Northeast Ohio.
NFP's staff and board maintains a strong focus on preventative care, treating chronic disease, mental health and substance abuse issues, and takes a highly collaborative approach to solving health care and quality of life issues. Neighborhood Family Practice has collaborative relationships with various hospitals, including MetroHealth and Cleveland Clinic. Our electronic medical records (EMR) system allows for data sharing with our affiliated partners resulting in better coordination of care for our patients. We are a Federal Tort Claims Act (FTCA) Deemed Facility.
